Modified Intravesical Oxybutynin Chloride Therapy in Children with Neurogenic Bladder: a Ten-year Follow-up

چکیده
Hypothesis / aims of study Children with spinal cord disorders can present with neurogenic bladder, a condition in which the bladder partly or completely loses its ability to store urine and void at low pressure. A bladder with low compliance may cause urinary incontinence, which negatively impacts quality of life and renal function. Long-term high pressure neurogenic bladder can increase the risk of deterioration in renal function. Anti-muscarinic pharmacotherapy with clean intermittent catheterization (CIC) is currently considered one of the most effective treatments for these patients. However, some patients do not respond to oral medication or have unacceptable adverse events, e.g., dry mouth, constipation, drowsiness, and cognitive dysfunctions, which may result in medical withdrawal for these patients. Intravesical oxybutynin is an effective treatment with less adverse events compared with oral medication. There is increasing evidence that intravesical oxybutynin is an effective therapy against neurogenic bladder [1]. However, an important issue with this treatment is retention of the solution in the bladder. A previous study reported that intravesical oxybutynin with hydroxypropylcellulose (HPC) can effectively treat children with neurogenic bladder [2]. In the present study, we report on the efficacy, safety, and side effects of long-term modified intravesical oxybutynin therapy in children with neurogenic bladder.

Treatment with modified intravesical oxybutynin chloride for neurogenic bladder in children.
OBJECTIVE We have previously reported that intravesical oxybutynin chloride with hydroxypropylcellulose (modified intravesical oxybutynin) is an effective therapeutic agent for patients with detrusor overactivity. In this study, we report on the efficacy, safety and side effects of modified intravesical oxybutynin administration in children with neurogenic bladder. PATIENTS Modified intravesi...
متن کاملIntravesical Oxybutynin for Neurogenic Bladder in Children
Neuropathic bladder dysfunction caused by spinal cord disease may lead to irreversible renal damage and urinary incontinence. The majority of affected children can be successfully managed with standard medical treatment such as oral anti-cholinergic medication (typically oxybutynin or tolterodine) with or without Clean Intermittent Catheterization (CIC). The efficacy of this treatment, however,...
متن کاملIntravesical oxybutynin for children with poorly compliant neurogenic bladder: a systematic review.
PURPOSE Children with neurogenic bladder and poor bladder compliance are usually treated with bladder catheterization and oral anticholinergic medication. They may become nonresponders to the drug or present with severe side effects. We evaluated the effectiveness and tolerability of intravesical oxybutynin in children with poorly compliant neurogenic bladder. MATERIALS AND METHODS We conduct...
متن کاملLong-term outcome of intravesical oxybutynin in children with detrusor-sphincter dyssynergia: with special reference to age-dependent parameters.
AIMS Intravesical instillation of oxybutynin is an accepted and effective treatment in children with neuropathic bladder-sphincter dysfunction, when oral oxybutynin results in inadequate suppression of detrusor overactivity or intolerable side effects. However, as yet no data are available on long-term use and outcome. METHODS A patient cohort with detrusor-sphincter dyssynergia that started ...
متن کاملSuccessful treatment of hemorrhagic cystitis secondary to cyclophosphamide chemotherapy with intravesical instillation of prostaglandin F2 alpha.
The treatment of cyclophosphamide-induced hemorrhagic cystitis is difficult. We report a successful case of severe cyclophosphamide-induced hemorrhagic cystitis treated with intravesical instillation of prostaglandin F2 alpha. A 32-year-old woman underwent high-dose cyclophosphamide conditioning before the autologous bone marrow transplantation. She developed clot retention which required conti...
